Insurance verification is provided as a courtesy. Coverage information is based on what your insurance carrier tells us at the time of verification and is not a guarantee of payment. Final payment determinations are made by your insurance carrier after claims are submitted. Co-pays, deductibles, and out-of-pocket maximums are your responsibility.
