What Structured Outpatient is

Structured Outpatient (SOP) is our lightest level of care — designed specifically as a step-down after Intensive Outpatient (IOP). You attend one group session per week and meet one-on-one with your therapist once a week. That's it. Roughly two to three hours of clinical contact, on a schedule that bends around full-time work, school, and family life.

The goal isn't to add more treatment for the sake of it — it's to keep you connected to clinical support, sober peers, and a therapist who knows your story while you rebuild your routine in the real world.

How SOP works

Your weekly group is a small process and relapse-prevention group facilitated by a licensed clinician. It's a place to talk through what's actually happening in your week — work stress, family dynamics, cravings, wins, slips — and get real-time feedback from peers who understand. Your individual session is private one-on-one time with your therapist to go deeper on the things group can't cover.

Most clients stay in SOP for up to a year. Some step out sooner, others stay longer — the right length depends on you, not a calendar. We adjust the plan with you at regular treatment reviews.

Who Structured Outpatient is for

SOP is the right fit if you've completed IOP and want continued accountability without the time commitment of three sessions a week. It's also a good fit if you're early in recovery, stable, working full-time, and need a level of care that doesn't disappear the moment IOP ends. We do not recommend SOP as a starting level of care — most people need the structure of PHP or IOP first to build a real foundation.

How SOP fits the continuum

Altura's full continuum runs PHPIOP → SOP → alumni community. You can step up or step down at any time as life changes. Many clients begin in PHP or IOP, transition to SOP for up to a year as they rebuild life back at work, and then move into our alumni community for long-term support. If something gets harder, we can step you back up — there's no shame in that, it's how outpatient care is supposed to work.
